How to track medical expenses, HSA, and FSA in Expense Trail
Use wallets, categories, tags, and goals to manage HSA/FSA balances, deductibles, and reimbursable medical bills.
Medical spending is spread across multiple accounts—HSA, FSA, regular checking—and arrives as bills, EOBs, and insurance statements weeks apart. Expense Trail lets you track medical expenses, HSA, and FSA with manual wallets, fine-grained categories, receipt attachments, and a deductible savings goal. No bank linking, no OCR, no tax advice—just an honest ledger you control.
Before you start
You need at least one wallet created and a few spare minutes to set up categories. Guest mode works without an account, but sign in if you want cloud sync across devices.
Expense Trail is not tax software. For HSA qualified expenses, IRS rules, or FSA deadlines, consult a tax professional or your plan administrator.
Steps
Create an HSA or FSA wallet. Go to Wallets → Add wallet, name it "HSA" or "FSA", and enter your current balance. Update the balance manually after each qualified purchase or after employer contributions post.
You should see: A dedicated wallet that reflects your tax-advantaged account balance separately from checking.
Create medical categories. Under Settings → Categories, add categories for each expense type you want to separate.

You should see: Spending reports broken down by medical type rather than one large "Health" bucket.
Log medical bills from the correct wallet. When adding a transaction, select the wallet that actually paid: HSA for qualified expenses, checking or credit for everything else. Choose the matching category from step 2.
You should see: HSA wallet balance decreases; regular wallet stays untouched for non-qualified costs.
Attach the receipt or EOB. On the transaction detail screen, tap Attach and add a photo or PDF of your Explanation of Benefits or receipt. See attach receipts to expenses for the full walkthrough.
You should see: A paperclip icon on the transaction row confirming the file is saved.
Tag reimbursable expenses. Add a tag such as hsa-reimburse to any out-of-pocket expense you plan to claim back from your HSA later. Filter by that tag when it is time to submit claims.
You should see: A filtered list of every expense eligible for reimbursement in one tap.
Set a savings goal for your annual deductible. Go to Goals → Add goal, set the target amount to your plan deductible, and link it to a wallet. As you log medical bills, manually update the goal progress to see how close you are to meeting the deductible.

Common mistakes
- Logging HSA purchases from your checking wallet — Always pick the HSA wallet for qualified expenses so balances stay accurate.
- Skipping receipt attachments for HSA expenses — The IRS requires documentation for HSA withdrawals; attach the EOB or receipt at the time of logging.
- Forgetting employer HSA contributions — When your employer deposits funds, update the HSA wallet balance manually or log it as income into that wallet.

Can I track HSA contributions from my employer?
Yes. Log the contribution as income into your HSA wallet. This keeps the wallet balance accurate without distorting your spending categories.
How do I log an insurance reimbursement?
Log the reimbursement as income into the wallet that originally paid the bill. Add a note linking it to the original expense so you have a clear paper trail.
Can I export my medical expenses for my accountant?
Yes. Use export for accountant to generate a CSV filtered by category or date range. Your accountant can then reconcile HSA-eligible items against your tax forms.
Does Expense Trail link to my HSA provider or insurance?
No. Expense Trail is manual entry only. You enter balances and transactions yourself; there is no automatic feed from your HSA custodian or insurer.
What if an expense is partly reimbursed by insurance?
Log the full bill as an expense first. When the insurance payment arrives, log that amount as income into the same wallet and note the original transaction in the description.
Is there a way to separate FSA from HSA?
Yes—create a separate wallet for each account. Because FSA funds typically expire at year end, you can also set a spending goal in Goals to remind yourself to use the balance before the deadline.
